심폐소생술 교육 효과에 대한 메타분석 연구

Abstract

Purpose

This study aimed to identify the effects of cardiopulmonary resuscitation (CPR) training using a meta-analysis by effect size.

Methods

The effect sizes for each variable and the overall effect size for the collected data were identified. The homogeneity verification of the effect size and the difference among the average effect sizes for each mediation variable were determined.

Results

The overall average effect size for CPR training was 1.747. Homogeneity verification of the overall effect size was a Q-value of 3716.962, which was statistically significant (p=.000) when α=.05. CPR training showed statistically significant differences depending on age (p=.002), sex (p=.006), number of trainees (p=.000), research design (p=.000), training method (p=.027), and practical training tools (p=.000).

Conclusion

CPR training can effectively improve knowledge, skills, and attitudes about CPR. The results of this meta-analysis contribute to the development of more effective educational guidelines for future CPR training and the advancement of the CPR education field.

Ⅰ. 서 론

1. 연구의 필요성

현대 사회가 급격히 발달하고 산업화와 경제화가 진행됨에 따라 사고 및 재해의 발생 수가 늘어나고 있으며, 이에 따른 사망률 또한 지속적으로 증가하고 있다. 또한 국가의 빠른 경제성장으로 인해 국민들의 생활수준이 향상되었고, 식습관 및 생활습관이 변화하여 심혈관질환이나 뇌혈관질환, 고혈압, 당뇨와 같이 급성 심정지를 유발할 수 있는 위험한 질병의 발생률도 증가하였다. 통계청이 발표한 2015년 사망원인 통계에 따르면, 1위 암(76,855), 2위 심혈관질환(28,326), 3위 뇌혈관질환(24,455)으로 심혈관질환이 전체 사망자수(275,895)의 10.3%로 전년대비 0.4%가 증가하여 2위를 차지했다[1]. 급성 심정지를 유발하여 사망에 이르게 할 수 있는 질환이 증가하고 이에 따른 사망률 또한 증가하고 있는 반면, 심정지 환자의 생존 퇴원율은 2012년 4.4%로 매우 저조하다[2].
2015년 보건복지부가 시범사업으로 실시한 119 구급대원 등 1급 응급구조사 스마트 의료지도 시범사업 결과보고서에 의하면 사업기간인 2015년 8월부터 12월까지 교육받은 일반인의 심정지 환자를 대상 심폐소생술 시행 비율은 2014년 시범사업과 비교할 때 59.5%에서 71.3%로 11.8% 증가하였다. 현장에서 119구급대원들이 전문소생술을 실시한 결과 심정지 환자 병원 전 소생률은 평상시 7.7%에서 20.6%로 3.1배 증가하여 심정지 환자 발생 시 목격자의 심폐소생술 시작과 현장의 구급대원들의 전문소생술이 소생률 증가의 중요한 변인이었다[3].
심폐소생술에 대한 중요성이 대두되면서 미국 심장협회(American Heart Association, AHA)에서는 기본인명소생술 자격증(basic life support provider, BLS provider) 과정을 통해 의료인뿐만 아니라 일반인 또한 심폐소생술 교육을 받을 수 있도록 하였다. 그리고 기본인명소생술 강사(BLS instructor) 교육 시스템을 통해 심폐소생술 교육 인력을 확대하고 있다. 따라서, 심폐소생술 교육이 다양한 방법으로 시행되어 왔고 학계에서는 그 효과에 대한 연구를 많이 수행하였다. 그러나 각각의 교육 효과에 대한 연구 결과들을 메타분석 방법으로 객관적으로 비교하거나 요약한 연구는 찾아보기 어려우며 대부분 연구들은 각각 단순히 교육 효과를 서술하는 정도에 그치고 있는 실정이다. 또한 측정도구나 방법 등의 상이함으로 인하여 심폐소생술 교육 효과에 대한 각각의 연구결과를 객관적으로 비교하기 어렵다. 따라서 심폐소생술 교육의 효과크기를 객관적으로 비교하고 종합적으로 분석하여 그 효과를 측정할 수 있는 메타분석과 같은 근거 기반의 분석적 접근이 필요하다.
메타분석은 모수를 더 정확하게 추정하고 다중결과를 분석하고 평가할 수 있다는 점과 결과에 영향을 줄 만한 요인을 분석할 수 있다는 점과 오류와 왜곡을 최소화할 수 있다는 점에서 그 가치를 인정받고 있음에도 불구하고 메타분석을 활용하여 심폐소생술 교육 효과에 관한 연구들을 비교분석한 연구가 적다. 따라서 심폐소생술 교육 효과를 검증한 기존의 연구결과들을 객관적으로 비교분석하고 그 결과를 종합적으로 분석하여 교육 효과크기를 증대시킬 수 있는 요인들을 파악하여 심폐소생술 교육 프로그램 계획 시 적용하고 지침으로 만들어 효과크기가 검증된 교육방법 활용과 심정지 환자 소생을 높이고자 본 연구를 수행하게 되었다.

2. 연구의 목적

이 연구는 메타분석을 이용하여 심폐소생술 교육 효과에 관한 연구들을 객관적으로 비교분석하여 종합적 결론을 도출하고 추후 심폐소생술 프로그램의 개발 및 실시를 위한 교육 효과크기를 증대시키는 교육방법과 중재변인 분석 연구에 기초자료를 제공하며 구체적인 목적은 다음과 같다.
첫째, 심폐소생술 교육 효과에 영향을 주는 중재변인에 따른 효과크기를 분석하고 어떤 변인이 심폐소생술 교육 효과를 높이는데 영향을 주는가를 파악한다.
둘째, 심폐소생술 교육이 종속변인(지식, 술기, 태도)에 대한 효과크기를 비교한다.
셋째, 심폐소생술 교육 효과에 대한 평균 효과 크기를 비교, 분석하여 추후 심폐소생술 교육에서 부족한 부분을 개선하여 교육 효과를 높이는 방안을 제시한다.

Ⅱ. 연구방법

1. 연구설계

본 연구는 심폐소생술 교육 효과에 대한 논문 중 심폐소생술 교육을 독립변인으로 하여 종속변인의 효과를 검증한 석·박사 학위논문과 학술지에 게재된 논문을 대상으로 메타분석을 실시하였다.

2. 연구대상 및 자료 수집

1) 대상논문 선정기준

본 연구에서는 메타분석 대상 논문의 선정기준을 연구대상자(P: population or participants), 개입방법 및 프로그램(I: intervention), 비교집단(C: comparison), 연구결과(O: outcomes), 연구유형(S: study designs)을 의미하는 PICOS 기법을 준수하였다[4].
첫째, 연구 대상자(P)는 어떤 집단을 대상으로 연구할 것인가에 대한 것으로, 본 연구에서는 연령대별로 초·중·고등학생, 대학생, 성인을 대상으로 하였다. 심폐소생술은 보건의료행위에 해당하므로 심폐소생술 교육 방법을 달리하여 보건의료인 혹은 일반인을 대상으로 시행된다. 따라서 본 연구에서는 교육대상을 보건의료인, 일반인으로 구분하였다.
둘째, 개입방법 및 프로그램(I)은 어떤 중재에 대한 연구인가에 대한 것으로, 본 연구에서는 심폐소생술 교육을 독립변인으로 그 효과를 검증하고 본 연구에서 선정한 각각의 중재변인(교육대상연령, 성별, 교육대상, 프로그램 참여 인원수, 교육시간, 연구설계, 교육방법, 실습방법, 출판유형)에 대한 효과 또한 검증할 수 있는 실험연구 혹은 유사실험연구 논문을 분석대상으로 선정하였다.
셋째, 비교집단(C)은 중재를 가한 집단이 더 좋은 결과를 나타내는가를 알아보기 위해 어떠한 비교대상을 사용하였는가에 대한 것으로 본 연구에서는 실험집단과 통제집단이 모두 존재하는 논문과 단일 실험군을 대상으로 시행한 논문을 모두 분석대상으로 선정하였다.
넷째, 연구결과(O)는 중재의 효과로 무엇을 보고자 하는가에 대한 것이다. 본 연구에서는 심폐소생술 교육의 효과를 검증하기 위해 그 결과로 통계적 수치(실험집단과 통제집단의 평균 및 표준편차, 사례 수)가 정확히 제시된 논문만을 선정하였다. 예를 들어, 본 연구의 중재변인 중 하나인 교육시간에 대한 데이터를 제시하지 않았거나 사후검사의 평균과 표준편차를 제시하지 않은 논문은 제외하였다.
다섯째, 연구설계 유형(S)에 대해서는 실험연구 혹은 유사실험연구를 선정하였고, 단일집단을 대상으로 한 연구 또한 분석대상 논문으로 선정하였다. 하지만 결과에 있어 단순빈도분석으로 통계처리하여 데이터를 나타낸 논문은 제외하였으며, 사례연구, 문헌연구, 질적 연구 등 다른 연구방법을 적용한 연구도 제외하였다.

2) 자료수집 방법

본 연구에서는 분석대상 논문의 선정을 위해 PICOS에 따라 대상논문의 선정 기준을 정하고 자료를 수집한 후, PRISMA flowchart에 따라 최종 연구 선정과정을 진행하였다[4]. 메타분석에 필요한 자료의 수집을 위해 한국교육학술정보원(http://www.riss.kr), 국회전자도서관(http://dl.nanet.go.kr), 국립중앙도서관(http://www.nl.go.kr), 한국학술정보(http://kiss.kstudy.com), 디비피아(http://www.dbpia.co.kr)를 주로 이용하였으며, 웹을 통해 원문보기가 되지 않거나 초록만 있는 경우 우편복사신청 혹은 상호대차 방법을 통해 자료를 수집하였다.
또한 메타분석에 필요한 논문을 추출하기 위해 ‘심폐소생술’, ‘심폐소생술 교육’, ‘심폐소생술 프로그램’, ‘심폐소생술 교육 프로그램’을 주제어로 검색하였으며, 검색을 통해 수집한 자료는 총 1,645편이었다. 또한 수작업을 통해 발견한 12편의 자료를 포함하였다.
1차로 수집한 1,657편의 논문 중 중복 연구를 제외한 자료는 총 633편이었다. 이후 본 연구의 주제와 관련이 없는 연구를 제외하기 위해 각 논문의 제목 및 초록을 중심으로 검토하였으며 그 결과 542편의 논문이 제외되었다. 또한 평균과 표준편차 등의 통계치가 누락된 31편과 본 연구에서 선정한 중재변인에 대한 데이터가 누락된 4편을 제외하여 최종적으로 56편의 논문을 분석대상으로 선정하였다.

3. 분석방법

본 연구의 부호화를 위한 매뉴얼을 바탕으로 대상 연구물에서 데이터를 추출한 후 코드화된 자료를 CMA(comprehensive meta analysis) 3.0 version 프로그램을 이용하여 각 연구의 효과크기를 산출하였으며 전체효과크기와 95% CI는 랜덤효과모형을 적용하였다. 또한 동질성 검증은 Q 통계치와 I2 통계치를 이용하여 검증하였으며 효과 크기들이 이질적인 것으로 나타날 경우에는 중재변인별 효과크기의 차이를 검증하였다. 중재변인별 효과크기의 차이는 One-way Meta Anova를 통해 검증하였다. 출판 편향성과 관련된 통계치는 CMA 3.0 version을 이용하여 분석하였다. 분석은 학위논문 12편과 학술논문 44편을 대상으로 하였으며, 총 127개의 효과크기를 산출하였다.

1) 효과크기의 해석

효과크기의 변산도는 표본수에 영향을 받으며 표본수가 적을 경우 효과크기의 편포성이 낮아진다. 따라서 표본수가 적을 경우 효과크기의 편포성을 교정하기 위해 효과크기에 교정요인을 곱하여 Hedges’ ɡ로 나타낸다[5]. 본 연구에서 메타분석에 이용한 연구물은 각기 다른 수의 대상을 가지므로 모두 Hedges’ ɡ로 전환하여 분석할 필요가 있다[4].
본 연구에서의 효과크기는 Muller[6]의 연구에서 제시된 Cohen[6]의 기준을 근거로 해석하였다. ‘ES≤.20’은 매우 작은 효과크기, ‘.20-.50’=작은 효과크기, ‘.50-.80’=중간 정도의 효과크기, ‘.80-1.00’=큰 효과크기, ‘1.00≤ES ’일 경우에는 매우 큰 효과크기로 해석하였다.

2) 출판의 편향성

메타분석의 특성상 출판된 연구물만을 대상으로 분석하게 되므로, 동일 주제로 진행된 연구에 있어 출판되지 않은 경우 편향성에 대한 문제가 발생하게 된다. 따라서 본 연구에서는 이러한 문제점으로 인한 오류를 최소화하고 본 연구의 신뢰성을 확보하기 위해 Funnel plot, Egger’s regression intercept, Fail-Safe N, Duval and Tweedie’s trim and fill 방법을 이용하여 결과를 제시하였다.

Ⅲ. 연구결과

1. 심폐소생술 교육의 전체 평균효과크기

본 연구는 56편의 분석 대상 논문으로부터 127개의 효과크기를 추출하였으며 연구 대상의 특성은 <Table 1>에 있다. 이를 바탕으로 심폐소생술 교육의 전체 평균효과크기를 산출한 결과는 <Table 2>에 있으며 메타분석을 위해 선정된 연구물에서 추출된 개별 효과크기의 이질성을 파악하기 위해 I2검정을 실시하였다. 본 연구의 이질성 검정 결과에서 I2=96.610(p<.001)로, 이는 개별 연구물들이 96% 수준에서 이질적이라는 의미를 나타낸다. 따라서 고정효과 모형을 선택하는 것은 부적절하므로 랜덤효과 모형을 선택하여 효과크기를 산출하였다.
그 결과 심폐소생술 교육 프로그램의 전체 평균 효과크기는 ES=1.747로 나타났다. 또한 평균 효과크기에 따른 95% 신뢰구간은 1.541~1.952로 신뢰구간이 0을 포함하지 않으며, 유의수준 α=.05 수준에서 검증 결과 p값은 <.001로 나타나 통계적으로 유의하다고 볼 수 있다. 이는 Muller[6]의 연구에서 제시된 Cohen[6]의 기준을 근거로 매우 큰 효과크기(1.00≤ES)로 볼 수 있다.
또한 본 연구 결과에서 나타난 전체 평균효과크기를 U3 지수로 환산하면 96.00%로 나타난다. 이는 심폐소생술 교육 프로그램을 실시한 집단이 그렇지 않은 통제집단보다 46.00% 정도 효과가 있다고 해석할 수 있다.

2. 심폐소생술 교육 효과 연구의 출판편향성 검토

1) Funnel plot

Funnel plot은 효과크기에 대한 표준오차를 산포도로 나타낸 것으로, 본 연구의 Funnel plot은 <Fig. 1>과 같다. <Fig. 1>에 의하면 본 연구에서 산출된 효과크기의 분포는 비대칭 편향분포를 나타내어 다소 출판편향이 있는 것으로 판단할 수 있다. 이를 통계적으로 검증하기 위해 Egger’s regression intercept를 실시한 결과 회귀상수(intercept)=6.52, 표준오차(standard error, SE)=0.91, 95% CI=4.72, 8.33, t-value=7.16, df=125로 나타났으며 회귀상수는 p=.00000 수준에서 통계적으로 유의한 것으로 나타났다. 표준평균 차와 정밀성에 대한 Funnel plot은 <Fig. 2>와 같다. 이와 같은 결과로 보아 본 연구에서 분석에 이용된 연구들은 출판 편향성이 존재한다.
Funnel plot이 좌우대칭이 된다고 가정했을 때 효과크기를 추정하는 Trim and Fill 방법을 시행하였으며 그 값은 <Table 3>에 있다. <Table 3>에 의하면 관찰치의 효과크기와 보정치의 효과크기는 동일하게 높은 값을 보였다. 95% CI 역시 0을 포함하지 않아 통계적으로 유의한 결과였다.

2) Fail-safe N

본 연구에서 분석 대상으로 이용된 연구물들에 대한 출판 편향성을 검증하는 방법 중 하나인 Fail-safe N값의 산출 결과는 <Table 4>에 있다. 본 연구에서는 Orwin[7]의 공식을 이용하였다.
Rosenthal의 공식에 따르면 안정성 있는 안전계수 값은 (5N+10)보다 크게 나타나야 한다. <Table 4>에 따르면 본 연구에서 Orwin의 안전계수는 9,321이다. 이는 메타분석의 결론을 바꾸기 위해서 필요한 효과크기가 0인 유의하지 않은 연구의 수가 9,321편이라는 것을 의미한다. 이는 Orwin이 제시한 (5N+10)값인 290편보다 큰 값으로 안정성이 있다.
Orwin의 공식에 따라 연구 수(N)=56, 메타분석에서 나타난 전체 평균효과크기(d)=1.747, 효과크기(ES)의 최소 준거수준=0.20, 안전계수의 하한계=0으로 설정하여 안전계수를 산출하였다. 그 결과 전체 평균효과크기(d)를 최소 준거수준으로 낮추기 위해 필요한 연구 수는 575편이었다.
출판편향성과 관련하여 분석 결과를 종합해 볼 때, 본 연구에서는 출판편향성이 존재한다. 그러나 심폐소생술 교육 효과와 관련하여 본 연구에서 메타분석에 포함된 연구물의 수가 56편으로 한정되어 있음을 고려할 때, 575편의 연구물이 효과크기가 0이라는 이유로 출판되지 않았을 가능성은 현실적으로 매우 낮다.

3. 심폐소생술 교육의 효과크기와 95% 신뢰구간

본 연구에서 메타분석을 위해 최종 선정된 56편의 연구들로부터 총 127개의 효과크기가 산출되었으며, 이에 대한 표준오차(standard error, SE), 95% CI(confidence interval, 신뢰구간)을 그림으로 제시한 Forest plot은 <Fig. 3>과 같다.

4. 중재변인별 효과크기의 차이

심폐소생술 교육 프로그램의 효과를 측정하기 위하여 종속변인별 평균 효과크기를 분석한 결과는 <Table 5>와 같다.
교육대상연령별 평균 효과크기를 비교한 결과, 초·중·고등학생의 경우 효과크기가 ES=1.172, 대학생의 경우 ES=2.042, 성인의 경우 ES=1.469로 나타났으며 연령대와 관계없이 모두 높은 효과크기를 보이고 있다. 그 중에서도 대학생을 대상이 가장 높은 효과가 있고, 초·중·고등학생이 가장 낮은 효과가 있는 것으로 나타났다. 교육대상연령별 효과크기의 차이를 검증한 결과 Q-value=12.475, df=2, p=.002로 통계적으로 유의하였다.
각 연구 대상의 성별 효과크기를 비교하면 여자를 대상으로 한 경우 가장 큰 효과(효과크기 ES=1.865)가 있으며, 남자와 여자를 대상으로 한 연구의 효과크기는 ES=1.796, 남자를 대상으로 한 경우 가장 낮은 효과(효과크기 ES=0.315)가 있었다. 성별 평균 효과크기의 차이를 분석한 결과 Q-value=10.251, df=2, p=.006으로 나타났다. 따라서 교육대상자 성별의 효과크기는 통계적으로 유의한 영향을 주었다.
교육대상별 평균 효과크기를 비교한 결과, 보건의료인을 대상으로 한 연구들의 평균 효과크기는 ES=1.855, 일반인을 대상으로 한 연구들의 평균 효과크기는 ES=1.618로 모두 높은 효과크기를 보이고 있다. 교육대상별 효과크기 평균 간의 차이를 검증한 결과에 의하면 Q-value=1.248, df=1, p=.264로 통계적으로 유의한 차이는 없었다.
심폐소생술 교육 프로그램에 참여한 인원수별 효과크기의 평균은 최하 1.282 이상으로, 모두 아주 높은 효과크기를 나타내고 있는데 이를 심폐소생술 교육집단 크기별로 51명에서 70명으로 구성하여 진행하였을 때 효과크기가 ES=3.054로 가장 크고, 이어서 71명 이상으로 구성하였을 때 ES=2.000로 나타났다. 30명 이하로 구성하였을 때 효과크기는 ES=1.730, 마지막으로 31에서 50명으로 진행하였을 때 ES=1.282로 가장 낮게 나타났다. 심폐소생술 교육에 참여한 인원수별 평균 효과크기 간의 차이 검증 결과에 의하면 Q-value=25.445, df=3, p=.000으로 나타나 통계적으로 유의하다.
교육시간에 따른 평균 효과크기를 비교한 결과, 심폐소생술 교육을 301분 이상 진행하였을 때의 평균 효과크기가 ES=1.613으로 가장 낮았고, 181분에서 300분으로 진행하였을 때 ES=2.230으로 가장 큰 효과가 있었다. 교육시간별 효과크기 평균을 비교하기 위해 그 차이를 검증한 결과에 따르면 Q-value=5.725, df=2, p=.057로 통계적으로 유의하지 않았다.
연구설계별 평균 효과크기를 분석한 결과 단일군 전후 검사 설계를 적용한 연구물의 평균 효과 크기는 ES=0.958, 비교·대조군의 전후 검사 설계를 적용한 연구물의 평균 효과크기는 ES=2.677로 나타났다. 두 집단의 차이를 검증한 결과 Q-value=75.056, df=1, p=.000으로 나타나 통계적으로 유의한 차이가 있었다.
교육방법에 따른 평균 효과크기는 최하 1.235 이상으로 나타나 모두 높은 효과크기를 보이고 있는데 이들 교육방법을 세분하여 오프라인 방법으로 교육을 시행한 연구들의 평균 효과크기는 ES=1.850으로 나타났으며, 온라인 방법으로 교육을 시행한 연구들의 평균 효과크기는 ES=1.235로 오프라인 방법이 온라인 방법보다 높았다.
심폐소생술 교육방법별 평균 효과크기의 차이를 검증한 결과에 따르면 Q-value=4.906, df=1, p=.027로 통계적으로 유의하였다.
실습방법에 따른 심폐소생술 교육 효과크기의 평균은 마네킨 직접실습의 경우 ES=2.621로 가장 높았고, 이어서 PWW(Practice while watching)가 ES=1.405였다. 동영상 자가 학습의 경우 ES=1.240, 미상의 경우 효과크기가 ES=0.825로 가장 낮았다. 집단 간의 평균 효과크기 차이를 검증한 결과에 따르면 Q-value=33.111, df=3, p=.000으로 실습방법 변인은 평균에 통계적으로 유의한 영향을 미치는 것으로 나타났다.
출판유형별 효과크기의 평균을 비교한 결과 학위논문의 경우 평균 효과크기는 ES=2.111, 학술논문의 경우 평균 효과크기 ES=1.636으로 출판유형에 관계없이 높은 효과크기를 보이고 있다. 출판유형에 따른 심폐소생술 교육 프로그램의 평균 효과크기의 차이를 검증한 결과 Q-value=3.679, df=1, p=.055로 통계적으로 유의한 차이가 없었다.

5. 종속변인별 효과크기의 차이

<Table 6>에서 심폐소생술 교육 프로그램의 효과를 측정하기 위하여 종속변인별 평균 효과크기를 분석한 결과 최하 1.260 이상으로 나타나서 종속변인 모두 높은 효과크기를 보인다. 심폐소생술 술기의 평균 효과크기는 ES=2.497로 가장 높은 효과를 보였으며, 이어서 심폐소생술 지식의 평균 효과크기가 ES=1.680, 마지막으로 심폐소생술 태도의 평균 효과크기는 ES=1.260으로 가장 낮았다.
종속변인에 따른 평균 효과크기의 차이를 검증한 결과 Q-value=21.341, df=2, p=.000으로 나타나 통계적으로 유의한 차이가 있었다.

Ⅳ. 고 찰

심폐소생술 교육 프로그램의 전체 평균 효과크기는 1.747로 나타났으며, 이는 95% CI에 의하면 하한값 1.541, 상한값 1.952로 통계적으로 유의한 결과였다. Muller[6]의 연구에서 제시된 Cohen의 효과크기 해석 기준을 근거로 볼 때 이는 매우 큰 효과크기이다. 지금까지 수행된 선행연구를 종합해보면 심폐소생술 교육 효과에 대한 메타분석 연구는 수행된 바가 없어 본 연구 결과에서 나타난 효과크기를 타 메타분석 연구와 비교할 수는 없다. Lee 등[8]은 특정분야 및 특정 주제에 대한 연구논문 분석과 평가는 불필요한 반복연구를 지양하고 선행연구들을 정렬시키며, 후속 연구의 방향을 제시해 줄 수 있다고 하였다. 따라서 본 연구 결과를 근거로 향후 더욱 효과적인 심폐소생술 교육에서 지식, 술기, 태도를 증진시킬 수 있는 다양한 프로그램이 개발되고 실행되어야 한다.
본 연구에서 분석에 이용된 127개 효과크기 사례 수의 동질성 검증결과에 의하면 심폐소생술 교육 효과를 나타낸 효과크기 사이에는 통계적으로 유의한(p=.000) 차이가 있음이 나타났다. 이는 효과크기의 이질성을 나타내는 중재변인이 존재할 수 있음을 시사한다. 따라서 본 연구에서는 중재변인을 선정하여 이들이 효과크기의 차이에 영향을 주는가 여부를 분석하였다.
첫째, 심폐소생술 교육 프로그램은 대학생(ES=2.042)에 준하는 학력 및 연령층의 교육대상자에게 가장 효과적이며, 교육대상자의 성별 효과 크기에서는 남자와 여자를 함께 교육한 집단(ES=1.796)보다 여자(ES=1.865)를 대상으로 교육한 집단이 효과가 있었다. 교육생 수에 대한 기준은 미국심장협회와 대한심폐소생협회에서는 BLS provider 교육 시 강사 1인당 교육생 수는 최대 6명으로 제한하고 있고[9], 교육부에서도 일반 강좌에서 20명 미만의 강좌 운영을 권장하고 있다. 그러나 본 연구에서는 교육생 수가 51명에서 70명(ES=3.054) 집단으로 진행하는 것이 효과적으로 나타났으며 이는 본 연구 분석에 활용된 논문들이 연구 설계 시 국내 교육실정에 따라 1회 교육 시 교육생 수가 30명 이하의 논문들보다 51명에서 70명으로 선정한 연구들에서 보다 많은 수의 교육생들이 교육효과를 수적으로 보다 많이 긍정적으로 보여주어 교육의 효과크기가 더욱 컸다. 일반적으로 최대 교육생의 수를 선정하는 기준은 교육과정의 특성, 강사의 경험과 교육생의 특성, 장비의 개수 등 여러 가지 상황에 따라 다르다. 특히 실습교육과정에서 교육생의 수가 너무 많을 경우 서로간의 상호작용이 원활하게 이루어지지 않을 가능성이 높기 때문에 강사 1인당 학생수를 적정하게 제한할 필요가 있으며 심폐소생술 교육과정에서는 더욱 강조할 필요가 있다. 본 메타분석에 활용된 연구들의 프로그램 참여 인원수 사이의 평균 효과 크기를 비교한 결과 p=.000으로 통계적으로 유의한 수준에서 집단 간 차이가 있는 것으로 나타났으나, 본 연구에서는 강사 1인당 교육생의 수가 아닌 전체 교육에 참여한 인원을 조사하였으므로 이와 같은 점으로 미루어 볼 때 단순히 프로그램에 참여한 교육생의 수가 아닌 강사 1인당 교육생의 수를 연구문헌상에서 확인해볼 필요가 있으며 추후 결과 해석에 주의할 필요가 있다.
1회 교육시간은 180분에서 300분 사이(ES=2.230)로 진행하는 것이 효과적이며, 교육방법은 오프라인 교육을 통해 강사와 교육생의 면대면 이론 및 실습 교육이 이루어지도록 하는 것(ES=1.850)이 가장 효과적이었다. 오프라인 교육 시 실습방법은 강사의 시범과 마네킨을 이용한 교육생의 직접실습 방법을 이용하는 것(ES=2.621)이 효과크기가 가장 컸다. 최근에는 다양한 방법으로 심폐소생술 교육이 이루어지고 있는데, 이론을 바탕으로 한 주입식 교육, 인터넷을 통한 자가학습 교육, 마네킹을 대상으로 실습을 통한 교육 등 여러 가지 방법이 있으나[10], 교육의 효과를 증대시키기 위해서는 반드시 심폐소생술에 대한 이론과 실습 교육을 병행해야 하는 것을 강조해야 한다. 또한 교육 프로그램을 계획하고 설계할 때에는 본 연구결과에서 효과크기가 크게 나타난 중재변인에 따라, 즉 교육대상의 연령대는 대학생에 준하는 학력 및 연령층으로, 교육생 수는 51명에서 70명으로, 교육방법은 이론과 실습 교육을 병행하여 오프라인 면대면 교육으로, 실습방법은 강사의 시범과 마네킨을 이용한 직접 실습 등과 같이 본 연구 결과를 고려하여 심폐소생술 교육 프로그램을 계획해야 함을 의미한다.
둘째, 교육대상을 보건의료인(ES=1.855)과 일반인(ES=1.618)으로 분류하여 교육 효과의 크기를 비교했을 때 통계적으로 유의한 차이가 없다고 할지라도 두 그룹 전부 높은 효과크기를(ES=1.6이상) 보였는데, 두 그룹 간에는 세계소생지침 근거 심폐소생술 교육내용과 심폐소생술 방법에 차이가 있으므로 표준화된 세계소생지침에 따라 대상에 맞는 적합한 교육 프로그램이 적용되어야 한다.
셋째, 본 연구에서 심폐소생술 교육은 지식, 술기 및 태도를 측정하는 도구가 다양함에도 불구하고 종속변인인 대상자들의 지식, 술기, 태도의 효과크기(ES=2.497, ES=1.680, ES=1.260)가 모두 그 효과를 향상시키는 것으로 입증되었고 변인들 중에도 특히 심폐소생술 술기의 평균 효과크기(ES=2.497)가 가장 높은 효과를 나타내고 있어 심폐소생술 교육에서 이론과 함께 반드시 술기교육을 병행하는 것이 교육의 효과를 증대시키기 위하여 필요하다.
또한 좀 더 분석 연구들을 고찰하여 보면 심폐소생술 교육 효과를 나타내는 종속변인, 즉 심폐소생술에 대한 지식, 술기, 태도에 대한 측정 도구가 다양함을 알 수 있어 향후 심폐소생술 교육 효과에 대한 연구 진행 시 심폐소생술에 대한 지식, 술기, 태도에 대한 표준화된 측정 도구를 개발하는 것이 마땅하다. 이는 측정 도구의 신뢰도와 타당도를 높이기 위해 필요한 선행 조치이다.
넷째, 앞에서 언급한 바와 같이 본 연구에서는 메타분석을 수행함에 있어 효과크기 사례수 조정을 위해 초등학생, 중학생, 고등학생 집단을 하나의 집단으로 하였다. 특히 선행연구에서는 심폐소생술 수행 능력에 있어 고등학생보다 초등학생이 뛰어나며[11], 초등학교 6학년에서 심폐소생술 교육 효과가 가장 높게 나타나는 등[12] 연령에 따라 차이를 보이는 것으로 나타났으며, 실제로 우리나라에서는 2009년 보건 교과 개설로 초등학교 5, 6학년과 중학교 및 고등학교 1학년 학생들에게 연간 17시간 보건 교육과정 내에서 심폐소생술 교육을 시행하고 있다[13]. 본 연구에서는 교육대상 집단을 초등학생, 중학생, 고등학생을 하나의 집단으로 분류하고 대학생 집단과 대학생을 제외한 성인 집단으로 총 세 개의 그룹으로 분류하여 메타분석을 한 결과 효과크기 사례수가 가장 많은, 즉 교육효과 측정시 종속변인인 지식, 술기, 태도에 대한 반응을 보다 세밀하게 측정한 연구대상 집단인 대학생 집단에서 심폐소생술 교육의 효과크기가 가장 컸다. 그러나 향후 연령대별 심폐소생술 교육 효과를 확인하기 위한 메타분석 연구를 진행할 때는 연령대에 대한 효과크기 사례수를 비슷한 수준으로 제한하여 교육의 효과를 검증할 필요가 있다.
본 연구는 국내에서 이루어진 심폐소생술 교육프로그램에 대한 효과를 보고한 연구 결과들을 메타분석을 통해 체계적으로 종합하여 분석한 최초의 연구라는 데 의의가 있으며, 본 연구 결과는 향후 심폐소생술 교육 분야의 발전에 이바지할 것이다.

Ⅴ. 결 론

1. 결론

심폐소생술 교육은 평균 효과크기 1.747의 매우 큰 정도의 효과를 가지고 있으며, 심폐소생술 교육을 실시하지 않은 집단보다 효과가 있다. 또한 심폐소생술 교육을 통해 심폐소생술에 대한 지식과 술기 능력, 심폐소생술에 대한 태도를 효과적으로 향상시킬 수 있다.
심폐소생술 교육은 대학생에 준하는 학력과 연령층을 대상으로 할 경우 가장 효과적인 것으로 나타났으며 여성을 대상으로 교육하는 경우에 높은 효과를 얻을 가능성이 크다. 또한 프로그램 참여 인원수를 51명 이상에서 70명 이하의 경우 심폐소생술 교육 효과를 극대화할 수 있다.
심폐소생술 교육은 심폐소생술에 대한 지식과 술기 능력, 심폐소생술에 대한 태도를 향상시키는데 충분한 효과를 기대할 수 있으며 특히 180분 이상에서 300분 이하의 교육 프로그램을 진행할 경우 높은 효과를 얻을 가능성이 크다. 또한 비교·대조군 설계를 이용한 연구에서 큰 효과를 보이는 것을 확인할 수 있다.
교육방법은 심폐소생술 이론 및 술기교육을 병행하는 것이 교육의 효과를 증대시킬 수 있으며 나아가 세부적인 방법에서는 오프라인에서 강사와 교육생의 면대면 교육을 진행한 경우에 높은 효과를 얻을 수 있으며, 실습방법도 강사의 직접실습을 통해 설명하고 마네킨을 이용한 실습을 진행하는 경우에 효과를 가장 극대화할 수 있으나 기본적으로 동영상을 보고 따라하는 방법의 실습을 진행하는 경우에도 비교적 높은 효과를 얻을 가능성이 있음을 나타내었다. 특히 심폐소생술 교육은 반드시 실습교육을 포함시켜야 술기 능력을 향상시키는데 높은 효과를 얻을 수 있음을 입증하였다.

2. 제언

본 연구의 제한점과 후속 연구를 위한 제언은 다음과 같다.
첫째, 본 연구에서 메타분석에 이용된 심폐소생술 교육관련 논문은 국내에서 출판된 논문으로 한정되어 있으므로 향후 국외 문헌을 포함한 연구가 이루어져야 할 것이다. 현재 시도되고 있는 국내의 심폐소생술 교육 효과를 살펴본 연구들이 축적되면 현재까지 국외에서 보고된 연구들을 포함하여 이들을 대상으로 교육의 효과크기를 측정하기 위하여 메타분석을 수행할 필요성이 있다.
둘째, 본 연구에서는 메타분석을 수행하기 위한 기초 작업으로 사례수 분배에 따른 중재변인의 유목화를 위해 초·중·고등학생을 통합하여 하나의 집단으로 선정하였으나 향후 연령대별 심폐소생술 교육 효과를 연구함에 있어서는 분류를 세분화한다면, 심폐소생술 교육을 어떤 연령대를 대상으로 실시하였을 때 가장 효과적인지를 밝힐 수 있을 뿐만 아니라 학교 교육에서의 심폐소생술 교육에 대한 실제적인 지침을 제시할 수 있을 것이다.
셋째, 본 연구에서 메타분석의 대상 연구들은 데이터베이스 검색과 수작업 및 회색문헌 검색을 이용하는 등 연구물을 확보하기 위한 최대한의 노력을 하였다. 그러나 미출판 연구들로 인한 출판 편향에 대한 오류를 배제할 수 없기 때문에 향후 이러한 점을 고려한 메타분석 연구가 이루어지기를 기대한다.
